1. Download the PDF ("Indemnification Agreement")
2. Print the PDF (recommended)
3. Read carefully!!
4. Sign the agreement OR sign the required areas digitally!
5. Scan & Upload the agreement to the Google Form! ("Volunteer Indemnity Form Submission")!

Make sure to rename the PDF to "[First Name][Last Name] - Indemnity Form". We hope to see you soon!
